Job Form To be filled out when you accept a co-op/internship

Learning Objectives--Logistics (PDF) (Word)

Learning Objectives--All other majors (PDF) (Word)

Student Evaluation --Completed at the end of each semester by the student.

Employer Evaluation --Completed at the end of each semester by the student's supervisor.

Mid-Semester Evaluation --Required for COMS 4800, DFST 4023, FINA 5800 and MGMT 4800/5800. Completed in middle of each semester by the student's supervisor.

Logistics Timesheet (PDF) Complete mid-semester and again at the end of each semester.
