After reviewing the information on the Family Educational Rights and Privacy Act of 1974, please...

1. Print the acknowledgment form.
2. Complete and sign the form indicating that you understand the information contained in this training session.
3. Fax the form to the Registrar's Office as indicated. This form can be read by using  Adobe Acrobat Reader.        
4. Active your EUID.

When the form is received in the Registrar's Office, EIS access will be assigned.
