Application Checklist

Application Packet Materials

• Applicant Form: Personal data, Academic data, Essay, and Contract. Please use a computer or typewriter to complete the form. The character size for all responses should be 12 pt. Proofread essays for correct grammar, spelling, and punctuation.

• Three letters of recommendation: Use the Recommendation form provided. At least one of the recommending parties must be a former or current science or mathematics professor. The recommendation form is confidential and needs to be returned in a sealed envelope with the recommender's signature across the seal. Include the sealed recommendations with your application.

• Transcripts: Provide current transcripts from each college/university you have attended. Copies of transcripts are available through the UNT Registrar's Office on the first floor of the ESSC. NOTE: Applicants not currently enrolled at UNT must provide official transcripts from each institution attended.
• Contact Information: Provide a minimum of three personal contacts knowledgeable of your whereabouts for a period of five (5) years upon and following University graduation. This information is strictly confidential and will only be used in the event that you are unable to be reached for NOYCE Scholarship Program tracking and statistical purposes.
• Application for financial aid (optional): Apply for federal financial aid and include a copy of the first two pages of your Student Aid Report (SAR) www.fafsa.ed.gov.
