Questionnaire
for the
National Information Standards Organization's Study of
State Library Agencies' Information Technology Standards Policies and Procedures

The study's first data collection activity involves distributing a questionnaire to all state library agencies to gather initial information about the agencies' standards policies and procedures. The study team distributed the questionnaire in paper form to the Chief Officers of State Library Agencies at the COSLA meeting on June 26, 1998.

The transmittal memo for the questionnaire is also available. The memo provides some information about the study and the focus of this preliminary questionnaire.

For agencies' convenience, the study team has an electronic version of the questionnaire in various formats. Agency staff responsible for completing the questionnaire can download a copy of the questionnaire to fill out in electronic form.

• Questionnaire in Word 97 (suitable for downloading and completing in electronic form: preferred)
• Questionnaire in Rich Text Format (RTF) (suitable for downloading and completing in electronic form: preferred)
• Questionnaire in ASCII (suitable for downloading and completing in electronic form)

For those respondents who complete an electronic version for the questionnaire:

• Save your completed questionnaire in Word, RTF, or ASCII text
• Attach the file containing the completed questionnaire to an email message
• In the Subject line of the email message, put Completed Survey [name of your state]
• Send the email message with attachment to: untniso@unt.edu
